ABOUT CASCADE:
Cascade has been providing treatment services to Lewis County and surrounding areas for more than 50 years. Our mission is to provide our clients with compassionate and encouraging counseling leading to a renewed sense of hope and the ability to make positive changes and live more satisfying lives. We currently have six treatment locations in Lewis County.
The Community Resource Specialist provides supportive, recovery-orientated transition services for clients actively working with the Jail Services team as directed by the Jail Services Manager.
Cascade Community Healthcare, as a health care facility, must comply with the Washington State Governor’s mandatory vaccination proclamation. The proclamation states all employees must be fully vaccinated against COVID-19 by October 18th. Exemptions from the vaccine requirement are allowed for those individuals who are entitled to a disability-related reasonable accommodation or a sincerely held religious belief accommodation.
WHAT YOU NEED TO GET THE JOB DONE:
High School Diploma or GED required, A.A. degree preferred.
Washington State DOH credential required (AAC).
Must be able to pass WA State Patrol background check. Also, a national background check if have not lived in WA state consecutively the last 3 years.
Must be able to obtain Lewis County Jail Clearance.
Two years experience working in a social service setting preferred.
Must be able to work as part of a coordinated team.
Must be able to work a flexible schedule.
Must have a valid driver’s license.
Must be able to operate a motor vehicle.
Must be able to gain access to all types of buildings and residences regardless of the structures’ handicapped accessibility or lack thereof.
Must maintain current home telephone and address on file with the personnel office.
WHAT YOU’LL DO:
Provides services for chronically mentally ill adults under the direction of the community support services clinicians/case managers including but not limited to:
Enroll qualified inmates in the WA Health Care Plan if they are within 30-days of a verifiable release date.
Coordinate services for utilization of the Lewis County Housing Resource Center.
Provide information regarding educational opportunities that are available within the jail and upon release.
Identify any inmates with a veteran status noted upon booking and coordinate available services with the local Veteran’s Affairs office.
Provide support and coordination of services with the Department of Child Support and Child Protective Services.
Provide access, on a case-by-case basis, to clothing and/or jackets for those who are in need and provide information for additional clothing supports available within the community following release.
Assist inmates in obtaining transportation passes for Lewis County destinations upon release.
Works as part of the Jail Services team to ensure case management needs are being met, assigned clients are being tracked; follow-up on referrals are completed; and effectively maintain standard quality of care to all clients.
Maintains clinical records and provides reports in accordance with agency policies and procedures
Works as part of the Jail Services team to provide liaison and consultation and educational services to other agencies or resources, as well as to others who are actively involved with the client (i.e., family, friends, etc.).
Ensures that ethical and professional standards of practice are maintained.
Participates in supervision, consultation, and training at the direction of the Jail Services Manager.
Provide other tasks and/or services as needed at the direction of the Jail Services Manager.
Provides reports in accordance with agency policies and procedures.
Ensures that ethical and professional standards of practice are maintained.
Participates in other activities at the direction of the Jail Services Manager.
