GENERAL SUMMARY:
Catholic Charities of Louisville’s Migration and Refugee Services (MRS) provides core resettlement services to refugees for up to 90 days and supportive services up to 5 years after their arrival in the United States. MRS also supports Cuban and Haitian immigrants and asylees. Recently resettled refugees and asylees often begin their new lives in the U.S. with no job prospects, little to no English skills, interrupted education, and limited understanding of U.S. cultural systems. The goal of MRS is to provide the resources and support to enable refugees and asylees to become self-sufficient. Our department also serves Latinx youth and their families through our Safe Release Services program, Home Safety/Post Release Services, and youth programs. In addition, Catholic Charities’ MRS partners with the Archdiocese of Louisville’s Office of Hispanic Ministry which connects the Latinx community with support services. The overarching goal for the AmeriCorps Healthcare Outreach initiative is to build the organizational capacity of the agency to serve Latinx and asylum-seeking clients through case management that addresses their most critical healthcare needs, including social determinants of health such as housing and education.

ESSENTIAL DUTIES AND TASKS:
The essential duties of the Immigrant Health Outreach Caseworker are:

Conduct intake and screening for potential clients in Spanish.
Build relationships with other social services, public, and community organizations in order to meet client needs.
Coordinate with local clinics and healthcare providers to schedule vaccinations and health screening appointments.
Assist or refer eligible clients in applying for Social Security cards, SNAP (Food Stamps), K-TAP (TANF), Medicaid, SSI or SSI-Disability, Housing Assistance.
Communicate effectively on behalf of clients with local public and service providers.
Educate clients in Spanish on local resources and empower clients to access resources independently.
Complete and update assessment tools and case plans used to measure clients’ progress.
Enter client case notes on a regular basis and maintain client case file in orderly and prescribed manner.
Complete regular program reports.
Other related tasks as assigned.

This position is a part of Catholic Charities USA’s Healthcare Outreach Program, a national intermediary project. As a member, you will meet virtually with other members serving throughout the country, all focused on assisting individuals within healthcare outreach.
WORKING CONDITIONS:
This work is performed in a busy office setting with moderate noise and activity due to computers, printers, phones ringing, clients entering and exiting the building and people moving about. Depending on agency and government COVID protocols, this position may require travel offsite to meet with partners and visit community agencies.

EDUCATION AND EXPERIENCE:

Must be at least 18 years of age.
Some college preferred.

PECIALIZED SKILLS AND KNOWLEDGE:

Spanish language proficiency required
Must be proficient in Microsoft Office programs, especially Word and Excel
Must be able to communicate verbally and in writing in an effective manner
Must be able to exercise good judgment and adapt to change
Must be people oriented and relate to a varied ethnic population
Must have excellent phone manners
Must be able to multi-task and work under pressure
Must be able to maintain the confidentiality of any information she/he encounters
Must be able to drive personal vehicle to various locations within the Louisville Metro area
